Chronic Disease Management

We have a strong commitment to the wellbeing of our local community, as such our patients that are suffering with a chronic medical condition can access management plans.

A chronic medical condition is one that has been present for at least six months or longer, such as:

• Asthma
• Cardiovascular disease
• Stroke
• Diabetes
• Cancer
• Musculoskeletal conditions

GP Management Plans are designed for patients who have complex conditions that require ongoing care, and a structured approach.

Our experienced GPs can determine if providing a management plan is appropriate for you.

Care plans offered to eligible patients suffering with a chronic disease incur no cost.

To access a care plan, patients are required to make an appointment with their GP. The GP will determine if the patient is eligible and if appropriate provide the plan.
