Director & Principle Practitioner
(MBBS, FRACGP)
Dr. Ameen Mohammed has over 25 years of experience in all aspects of general practice. He has an interest in rehabilitation, paediatrics, Chronic Disease Management, women’s and men’s health, minor surgery and ingrown toe nail treatment.
Dr. Ameen Mohammed encourages multiculturalism and actively participates in discussions with patients from various cultural backgrounds in order to understand and address issues specific to their cultural needs sensitivity.

Dr Megan Clark
Psychologist - BA Psych/Phys (Hons), MAPS (Clinical), PhD
Dr Meg is a general psychologist who is currently completing her Clinical Psychology registrar program. During her clinical training, Meg worked in community mental health and private practice. Meg has a passion for working with children, adolescents and their families with a particular interest in parent-child relationships, anxiety and emotion regulation difficulties. Meg provides evidence-based interventions for a broad range of psychological concerns including anxiety disorders, mood disorders, social communication challenges, relational and attachment difficulties. Meg has over 10-years’ experience conducting developmental and diagnostic assessments of common neurodevelopmental disorders including autism spectrum disorder and attention deficit hyperactivity disorder.

Mr Brayden Chung
(Occupational Therapist)
Brayden is a registered occupational therapist experienced in supporting children, teens, and adults with a range of physical, developmental, and psychosocial disabilities. With a background as both a therapist and long-term support worker, he brings a strong person-centred and neurodiversity-affirming approach to every interaction.
His career began at one of Western Australia’s largest NDIS providers, where he supported a diverse caseload across paediatrics and adult disability and gained valuable exposure to the NDIS framework. However, it was during his time working as a sole trader that Brayden truly honed his skills—particularly in writing high-quality Functional Capacity Assessments. In this role, he worked closely with individuals with ASD, ADHD, and psychosocial conditions, delivering tailored interventions that highlighted individual strengths and supported meaningful access to appropriate funding and supports, including Supported Independent Living (SIL).
Brayden has a passion for mental health and has supported clients living with anxiety, depression and other complex psychosocial conditions. This experience has shaped his calm, collaborative, and strengths-based approach—particularly in helping clients build structure, emotional regulation, and confidence in their daily lives.
Fluent in English, Cantonese, and Mandarin, Brayden is passionate about culturally responsive practice and forming strong partnerships with families. With close personal connections to neurodivergent individuals, he brings not just clinical knowledge but also lived empathy into his work.
Outside of therapy, Brayden enjoys staying active—he’s often found at the gym or learning new skills on the badminton and tennis courts. He brings the same curiosity, focus, and balance to his practice—whether he’s collaborating with a team or celebrating a client’s small win.

We know life gets busy and things happen so please let us know if you cannot make your appointment. This allows us to offer this appointment to someone else in need. If you are not able to make your appointment we require a minimum of 2 business days’ notice, otherwise a cancellation fee – 50% of the session fee – will be charged. Please note that weekends and public holidays are not business days. Failure to attend an appointment will attract the full fee and cannot be claimed via Medicare. Ms Gurpreet Kaur is a dedicated physiotherapist at CareFamilyPractice, committed to helping individuals move better, recover faster, and live pain-free. With years of experience in physiotherapy, she provides personalized and evidence-based care tailored to each patient’s unique needs.
Gurpreet specializes in the treatment of back, neck, shoulder, hip, and knee pain, as well as muscle and joint conditions. She also has extensive experience in managing sports injuries, post-surgical rehabilitation, work and vehicle injuries, and pelvic health physiotherapy. Her approach combines advanced techniques with compassionate care, ensuring effective recovery and long-term results.
